Focusing on a monthly payment is an approach to stop you from thinking about what? How can you combat this?

The total debt you are assuming. You should think about the total debt you are assuming; not the monthly payment.

Your simple and short answer is the correct answer, Katherine. It is critical to stop thinking in monthly payment terms if you ever want to get a handle on your debt. You MUST think in total debt assumed if want to be totally honest with yourself. This is something a business would probably not want you to do. You would end up buying less stuff on credit if you did. This would certainly hurt the business, but help you. Focus on fixing YOU and that means changing the way you think and ultimately behave.
